Long-Term Care Code
The Ministry of Health offers assistance in financing long-term nursing care for persons with physical impairments and persons with cognitive impairments according to the financial situation of the long-term nursing care candidates and their family members.
The Ministry of Health process of long-term nursing care consists of two stages:
- Determining the patient's medical and functional status and the suitability for care in a nursing facility for persons with physical impairments or persons with cognitive impairments.
- Determining the amount of copay for the long-term nursing care candidates and their family members in financing long-term nursing care.
Application
You can submit an application in two ways:
- Fill out and submit the online form.
- Print a form for manual completion and send it along with the required documents to the "Department of Chronic Diseases" at the Health Bureau in the patient's area of residence.
